Lezlie M. Shively, age 22, of Lewisburg, OH, died Wednesday, January 26, 2011 at Good Samaritan Hospital in Dayton, OH. Lezlie was born on September 5, 1988 in Dayton, OH and was a 2007 Tri County North graduate and attended C.T.C. She was a Volunteer at the Tri-County North High School Library.

(Marker) Shively-Hawley & Steve Hawley of Lewisburg, OH; Father: Ray A. & Lori Shively of Eaton, OH; Brothers: Chad A. & (Shannen) Shively; Kodi Hawley and Skyler Hawley all of Lewisburg, OH; Sister: Rachel Hawley of New Lebanon, OH; Paternal Grandparents: Lloyd & Glenna Shively of Lewisburg, OH; Maternal Grandparents: Devon & Carol Marker of Eaton, OH; Niece: Delaney Shively; Nephews: Owen Shively and Ryan Shively; Caregivers: Rachel Fields and Kristin Pettitt.

Funeral Service will be held 11:00 AM Monday, January 31, 2011 at Barnes Funeral Home – 411 North Commerce Street – Lewisburg, OH with Revs. Brent & Laura Cavendish officiating. Interment will be in Roselawn Cemetery in Lewisburg, OH. Family will receive friends on Sunday, January 30, 2011 from 3:00 to 6:00 PM at the funeral home.
